The following questionnaire will help you organize your thoughts about how you would like your project to proceed. You can save this page (File – Save As) in a file which can then be edited using FrontPage, Microsoft Word, WordPad or the Notepad Editor.

General Questions:

	What is your target date for completing your project?
	Do you have a color scheme in mind?
	Do you have any of your own photos, logos and art work?
	Do you have any of the text already written?

Web Site Questions:

	Do you have a Web Site now? What is its URL?
	Do you need a place to host your Web Site?
	What do you especially like about your current site?
	What things on your current site would you like changed?
	What other Web Sites have you seen that you particularly like?
	Have you developed a storyboard for your Web Site?

Print Project Questions:

	What kind of project do you need (business cards, greeting cards, invitations, brochures, forms, stationery, etc.)?
	Are you preparing for an event? If so, what is the theme of the event?
	Would you like a design-matched set of several of the above (e.g., matching stationery, envelopes and business cards)?
	Do you want your project delivered electronically, or do you want to receive the printed results?
	If printed, how many copies do you need?
	Do you have special requirements for the paper used?

Database Questions:

	What is the subject area of your database (Contacts, Orders, Events, Inventory, etc.)?
	Do you need an application to populate and maintain your data?
	Do you have any sample data, or do you need some sample data to be generated?
